Before scheduling, the clinic verifies location, clinician licensure, service fit, fees, and any applicable payer participation.

When scheduling opens, patients will use a designated secure system for intake forms, consent, and appointments.
04 · Attend your virtual session
Meet with your assigned clinician through the clinic's approved telehealth platform.

StarRising is developing contract-based care coordination solutions for health plans, healthcare providers and community organizations in Connecticut and New Jersey. We welcome discussions about vendor qualifications, pilot programs and partnership needs.
Member Outreach & Engagement
Nonclinical outreach, engagement attempts, appointment reminders and support connecting members to covered care.
Referral & Resource Navigation
Referral follow-through, scheduling assistance, community resource navigation and documented coordination with authorized care teams.
Transitions of Care Support
Nonclinical post-discharge outreach and appointment coordination under partner-defined protocols and escalation procedures.
Care Coordination Staffing
Potential dedicated care coordinator and care manager teams, subject to contract terms, role qualifications and appropriate supervision.

We do not list clinicians as available until their credentials, practice permissions, and onboarding requirements have been confirmed.
